- Personal information we collect about employees, job applicants, or independent contractors in their capacity as such.

In the last twelve (12) months, we have collected the following categories of Personal Information:

A. Disclosure for a Business Purpose
In the preceding twelve (12) months, we have disclosed the following categories of Personal Information for a business purpose to our service providers (e.g., website hosts, billing processors, appointment scheduling software):

- Customer Records (non-PHI)
- Internet/Network Activity
B. No Sale or Sharing of Personal Information
Family Care Podiatry does not sell your Personal Information for monetary or other valuable consideration. We also do not “share” (as defined by the CPRA for cross-context behavioral advertising) your Personal Information. We do not have actual knowledge that we sell or share the Personal Information of consumers under 16 years of age.
5. Your California Privacy Rights
If you are a California resident, you have the following rights regarding your Personal Information, subject to certain exceptions:
A. Right to Know
You have the right to request that we disclose what Personal Information we have collected, used, disclosed, and/or sold/shared over the past 12 months, including the categories and specific pieces of Personal Information.
B. Right to Delete
You have the right to request the deletion of Personal Information that we have collected from you, subject to certain exceptions (e.g., to complete a transaction, comply with a legal obligation, or detect security incidents).
C. Right to Correct
You have the right to request the correction of inaccurate Personal Information that we maintain about you.
D. Right to Opt-Out of Sale or Sharing
As noted above, Family Care Podiatry does not sell or share your Personal Information, so no opt-out is necessary.
E. Right to Non-Retaliation
We will not discriminate against you for exercising any of your CCPA/CPRA rights. This includes denying you goods or services, charging different prices, or providing a different level or quality of goods or services.

Verifiable Consumer Request
We must be able to verify your identity to fulfill your request. We will ask you to provide identifying information such as your name, phone number, and email address, and we will match that to the information we maintain. You may also designate an authorized agent to make a request on your behalf.
7. Data Retention
We retain each category of Personal Information for as long as is necessary to fulfill the purposes outlined in this Privacy Statement, or as otherwise required by law (such as mandatory medical record retention periods, which are generally governed by HIPAA).
8. Changes to Our Privacy Statement
We reserve the right to amend this Privacy Statement at our discretion and at any time. When we make changes, we will post the updated notice on our website and update the “Effective Date.”
